The information that Savy Kalivoda collects on her clients include name, gender, address, date of birth, home phone number, work phone number, cell phone number, occupation, email address, family doctor's name and phone number, and medical history.
Accountability: the privacy officer of this office is the owner and sole proprietor, Savy Kalivoda. The responsibility of the privacy officer is to respond to inquiries, complaints, update information, and understanding the legislation.

Safeguards: only Savy Kalivoda has access to the information and no unauthorized parties can access information.
